- Go to your nearest LabCorp at your convenience and if applicable your medication can be mailed directly to your home or electronically prescribed to your nearest pharmacy of choice.

After completing your intake and waiver/education verification, we will sign you into our secure, HIPAA-compliant electronic medical record, Practice Fusion. You will receive an invite to enroll in your private health record, Patient Fusion, where we will complete your plan for treatment!
After completing your intake form, our staff will process your request to determine eligibility and email you a waiver form with educational information. Please read this carefully and sign without obligation.
Upon payment, we will call in your prescriptions to your local pharmacy. For GLP-1 medications we will prepare your medication packet for pick up at the office in Belmont, NC for individual monthly doses. Medication administration/teaching will be provided on site and a link provided via the email waiver. Vials may be mailed directly to the patient’s residence.
